(S-COPE) is an innovative crisis response and clinical consultation team for adults age 55+, who reside in nursing facilities.


S-COPE operates as an interdisciplinary team consisting of a geropsychiatrist (consultants), an advanced nurse practitioner, registered nurse, licensed clinical social worker, and masters level clinicians with geriatric training.


S-COPE is funded by DOH and is operated by Trinitas Regional Medical Center.


S-COPE’s mission is to address psycho-geriatric challenges and provide professional training and staff development to insure the appropriate use of acute psychiatric services and effective support of older adults with behavioral and psychiatric challenges.


S-COPE goal is for older New Jerseysians who are residents within Long-Term Care facilities to receive the behavioral health supports they need to successfully age in place, and to collaborate with facilities to develop their expertise in promoting quality of life, wellness and dignity of each resident.

 

S-COPE is committed to promoting state-of-the-art geriatric behavioral health services within long term care facilities through consultation, education, on-site mentoring, and regional training.


S-COPE has 4 offices throughout the state and responds to crisis call 7 days a week.
